is it possible to send device location in a form?

I am creating an app and it will contain a form. Does anyone know of a free form generator that can gave a field that collects and sends device location?

That's exactly what this tutorial can do! https://www.buzztouch.com/files/howtos/Posting_Current_Location_to_a_Database_v1.0.pdf It sends it to a database ultimately, but shows you how to get the devices location into the form in the first place. Mark

Well, the tutorial actually uses a php script to grab the device's lat/long and build the form with it. Not sure if that's possible using Google forms or wufoo.

Not sure about Google forms but I know builders such as wufoo and jot form give you the HTML code so it can be customized so should be able to insert the code somewhere. Thanks I will look into it. Your help is appreciated as always.
