ERROR, could not initialize view controller for screen

When I select the appropriate menu item, my Feedback plugin screen should load, however nothing happens and the app just freezes. My debug log says... ERROR, could not initialize view controller for screen with itemId: 5D338D4A3F35AA7722D0994 Does anyone know what the problem is? I have tried deleting the project and downloading a fresh copy, but no luck. I can see the Feedback plugin in my BT_Plugins folder. I'm sure I've previously had this plugin working.
