I have 4 items in menu. when i test my app on simulator , it shows 3 items only but when i refresh the app it shows the full menu. What is the possible reason for it? Since then, you've added another menu item, but your BT_config.txt file still only shows 3. Until the app refreshes. At least, that's my first guess. update the BT_config.txt file in your project and I think you'll be ok.
