Hi FunkyMonkey. I actually got this plugin to work once, and what I found out was how limited it was in terms of creating fields, and extracting data. Even if you write a script for it, there are many limitations in terms of user experience. My solution to creating apps with "forms" is to sign up to 3rd party websites with mobile friendly solutions. Like FormStack or JotForm. Both I believe still offer a free account. I know this is not an complete answer to your question, but I hope it helps out in some way.

Hey FunkyMonkey, I use a form in my app for food reviews. What I wound up doing was creating a page in my apps wordpress site. I use the plugin Formidable Forms and just point the link for a food review to this page. I forget how much the pro version cost me but i can create any type of form that I want and the plugin keeps the submitted data in the plugin as well as email it to me . Good luck Dave

I have been making my forms with the customUrl screen. The webpages that I load are made with Twitter Bootstrap and are very pretty. I know a native form could be much better, at least theoretically. But it is so easy to add some nice style to a web form. If I didn't know how to make a form with web programming then I would use Jotform or Woofoo to make a form. I bet they make a beautiful responsive form. By the way, I have used Survey Monkey in apps with the customURL, and they work great.
