User types into a form

Hi, Which would be the best way (Maybe a Plugin?) to allow a user to type into a form field also have the ability to choose an option from a drop down menu. Than once completed they can email it, print it or save it to a database? Thanks

I was hoping for a funny ending too :)) @CoralSprings If you take the HTML route to solve this problem have a look at jquery mobile forms. There's some nice tutorials and videos out there to get you up and running. Cheers

Be careful guys, I looked at the form code example on angelfire dot com and clicked on a dropdown option on the form; it tried to launch some malware (which my AV trapped) - I believe that code has been compromised.
