A send button at the bottom could just send the filled in info to a predefined email address. (Still working on the Android version). It authenticates with Parse, Facebook or Twitter to have users 'login' to your app. I'm not making a plug-in out of it, but if you are interested in the iOS version and having someone add it to your app on a per project basis for a fee, shoot me a PM. It is currently March Madness (for non us folks - this is College Basketball tournament time, sorta like FIFA World Cup) and I'm deep in the midst of Bracketology and a handful of projects. So this won't be something I can add immediately. I would need to look at the structure of your app first as well before I can give you an ETA and quote.
