Create a form that is sent to my email

So i have had a lot of trouble with this issue. I want to create a form that people will choose boxes/multiple choose questions, and the results will be sent to my email. How would i approach/ make this?? Thanks, Jack

I use Wufoo. It has been awesome for me.

There are a few ways. I like to create the form then go to code and copy and paste the JavaScript Version into a Custom Html screen. It works great. However, you could just use the url link that wufoo provides and use a custom url screen.

I also have forms in one of my Apps check out jimmyscafe for iOS it's free if you like it I will share.
