I want to create a form that people will choose boxes/multiple choose questions, and the results will be sent to my email. How would i approach/ make this?? I like to create the form then go to code and copy and paste the JavaScript Version into a Custom Html screen. It works great. However, you could just use the url link that wufoo provides and use a custom url screen. Comments There are a few ways.
