Need help creating calculator screens with html

I am trying to create screens for my app similar to the following website: http://www.mdcalc.com/glasgow-coma-scale-score/ The user will be given options that they can click and then when done a final score with explanation will show. My app is for iOs and Android so prefer to stick to html/java. Anyone know any good resources for figuring this out? Could I use jquery for this? Thanks!! josh

It looks to be just javascript/html (jquery and some custom code), no php processing. You could pretty much get a working copy just be doing a File > Save Page As (firefox) then adapt to suit.
