Form to Email or SMS?

I'd like to be able to display an html form with a few fields and when the user hits submit, the form can go to a specific destination as an email and/or an SMS. I want to capture fields like drinks order, table number and send. Ideally this would work for android and apple. I've tried using a mailto form as an html/text screen but it doesn't work on my Android device, it pulls up a URL, not the mail program. Is this a tough ask?

not if you use jotform, give it a look.

Looks good and I have made a form, which worked from my PC - I haven't tried it as a custom URL but I guess that would be the next step. However, If I use Jot form, it looks like my form is hosted by them and I am not using the native phone features, which I would prefer to do.

I use Wufoo for the form in my app. It has the option of having the submit button send to both an email and sms destination simultaneously. They also look good on iPhone/Android and you can get rid of their logo on the form. Lanbito looks more natural for the iPhone (they specifically make it look like a native iPhone form) but it has fewer customizable options than Wufoo and you can't send the submit button to an sms.

Which option did you eventually go with? I'm looking to integrate a form in the email function and am trying to decide if I need to create a custom plugin. Thanks.
