Creating Forms

Hey, I have a client and he wants to have an appointment scheduling form for his business in the iphone app. He uses a system right now to book appointments from his website. What I want to do is create a form in buzztouch, not a web html page, but a form using iphone features, like this one here: http://itunes.apple.com/us/app/excuria-salon-and-spa/id381891221?mt=8 , if you look in the iphone screenshots you will see the form, whats cool about it is that you could click on pick and a bunch of options come up, their services, and then for the date, you click pick and all the months and days come up in a scroll wheel. So what I want to do is create a form like that, and integrate it into their system, so I would have to make all the fields/names/form locations the same. Is there anyone that knows of any plugins, or could direct me to where I can learn how to make a form like this using iphone features and not just a html page? Thanks!

I agree with MacApple, wouldn't be easy to make a custom html and embedded it into xcode. I know you said no html, but there are ways to make html really "pretty" and actually embedded it into xcode making it look like its part of ios software :)

You guys know of any form templates optimized for mobile devices? Edit: I found this site on the forums: http://www.jotform.com/, will try it out and see how it does.

Jotform doesn't seem to have a mobile friendly form. Its really small on the iphone and you have to zoom in to use it properly. I found that wufoo looks fine on the iphone, wufoo.com, but I rather find a template that I could use and process the form myself instead of using someone elses service.

To be honest with you, I have no idea what your client is using (as in current database). So can't really answer. What does he use to gather his calendar database?
