Creating Forms

Hey, I have a client and he wants to have an appointment scheduling form for his business in the iphone app. He uses a system right now to book appointments from his website. What I want to do is create a form in buzztouch, not a web html page, but a form using iphone features, like this one here: http://itunes.apple.com/us/app/excuria-salon-and-spa/id381891221?mt=8 , if you look in the iphone screenshots you will see the form, whats cool about it is that you could click on pick and a bunch of options come up, their services, and then for the date, you click pick and all the months and days come up in a scroll wheel. So what I want to do is create a form like that, and integrate it into their system, so I would have to make all the fields/names/form locations the same. Is there anyone that knows of any plugins, or could direct me to where I can learn how to make a form like this using iphone features and not just a html page? Thanks!

I think the first step is finding out what type of backend the client is using so you know what you'd be trying to connect to. After that, you'd want to use a custom plugin to connect to a custom class in xcode. I think it would be a lot of work, so make sure your client is willing to compensate you appropriately. :)

Woops, this forum post reposted after I refreshed. Well, the way it works is they have a service set up online where you can create an account if you are a first time booker, or login if you are a previous booker, what I can easily do is create a form in the app that would send all the details to their booking script and make it seem like the information was sent from their website pretty much, but once it logs in, it'll just load their web page and it'll be like booking online but from the phone. So nothing to difficult I don't think.
