Tried it on my live device and have the same results (iOS 8.1.3). Made all the changes as per PDF, cleaned and reinstalled - same difference :( Doesn't even prompt! It 'should work ok' with iOS 7 and prior. With iOS 8 though, you must have all that authentication stuff in there. Take another look and make sure all the details are covered.
