There are many PHP scripts to do that, but they require the php to reside on a server which supports it. Other languages can do it, but would have similar requirements. So you could use a URL screen, which points to a webpage having your form on it. Or you could do some magic with a plugin, or Obj-C programming.

I used a website called freedback.com . It is free for one form and there is an ad after the form is sent, but it's free and they give you the HTML code to drop into your BT HTML screen. After that I figured out that I could also make a feedback form in RapidWeaver (a Mac web publishing application selling for about $30??). This option means that you have to host the page online, but you then have FULL customization. Plus, to host the page, all you need to do is place it in the public folder in Dropbox, and boom, you're done!! The RapidWeaver option allows you to have the feedback form with no ads. Hope this helps!! Let us know how it goes!!!

I created a self hosted Wordpress website and setup a form using the plugin Contact Form 7 plugin. This works great and is free. I'm assuming you can still use this plugin on a Wordpress.com free site. I stripped down the Wordpress theme to show only the contact form and then created a webview that links to the page and removed web navigation buttons. The only drawback is that it requires an Internet connection to work. However, all solutions would require this.

Take the HTML code and insert it into a new HTML screen/plugin and go! From there you can change the HTML code to the layout format that best suites you. If you need any more help, let me know!!

I know it's been a while but I hope someone is still following this post. I also want to make an email form and I used jotform. They give you a short javascript line, or you can download the code with html, css, and js. how to implement all that in BT? html is easy but when it calls the src of css and js, where do I put those files?

Sorry, stupid question. I guess you can just put the css and js content inside the html document and upload it as a html doc. Nevermind. Hope that works like i want it to
