Making a custom form in BT?

Can anyone think of how a form could be made that would allow the user to capture the data in the boxes and then send to users desired email adress. Eg a gas engineer fill out a certificate of his work on hi s device then can email the results?There seems to be plenty of custom form makers out there but they all send to specific email adress. I just want a simple form with a few boxes and the data in those boxes to email to the users desired adress. I could create the form in HTMl 5 but getting the data form those boxes is getting me stuck....I tried a simple mailto: but the data just was illegible on the email. Perhaps there is a way to layout the captured data in a logical manner? eg: <form action="mailto:"" method="POST" enctype="multipart/form-data" name="EmailTestForm"> Your Name:<br> <input type="text" size="20" name="VisitorName"><br><br> Your Comment:<br> <textarea name="VisitorComment" rows="4" cols="20"> </textarea><br><br> <input type="submit" value="Email This Form"> </form> Results this on email: VisitorName=&VisitorComment= Id like to set the email out as: Visitors Name: Visitor comment:

Using the search engine on my site: www.myskylla.wordpress.com I searched the keyword 'form' The first item found offers a couple solutions. For more run the search. You can use a google form as well. Saves all the info to a spreadsheet, which you can then subscribe to.
