I have found a wonderful website that lets you create forms for free and you can do so much! I works really well with buzztouch because you can set the with to a mobile device and you just use the custom url/webpage feature in buzz touch and paste your link in. No ads!!!! The free account is 100 submissions per month and the premium or $9.95 a month or $90 a year is 1000 submissions per month. You should check it out!!! JotForm.com Let me know what you think!

Wouldn't an email signup form as created via an email marketing company like iContact also work?

@NDHMT, I created a form it Jorform in which i wanted my students to fill out the fields and view their document instantly. Is there a way that i can use the jotform service to have the users inputted information populate/generate a viewable document after submission? I tested it out and it looks like I, as the account holder can only view the submission via the site. Please advise
