Submitting to App Store

I feel like I've missed something, seeing as I can't find many forum posts about this. After you've made the app, how on EARTH do you submit it? I just find a bunch of conflicting guides and blog posts when I google it, and Xcode seems intent on making me uninstall it completely and do something else with me time. I have a provisioning code/license, yet I can't archive it. Just says failed. Naturally doesn't tell me where exactly the problem is other than code signing is required, even though I appear to have everything signed -___- Can anyone explain the required steps to upload? Please don't link and refer me to an old blog post. I want to know what YOU guys do to upload your apps!
