Vlad, Kindly read the forum guidelines and provide additional details because we need more info to help or at least point you in the right direction. What platform? What version are you building it in? Do you want it hosted on a server? or local in the app? You need the results to be counted and displayed? Sent to a database? Emailed to someone? Vlad, What you are asking to do is well out of my skill set. You might need to hire someone with php/database experience. I'm not sure about the best solution to accomplish your desired product.
