Both would use the custom HTML screen. 1- Create a email form with text fields, radio buttons, drop downs, etc. and mail it to an address to add the contact info by hand when it is received. 2- Have a form with the same info and instead of using the mail program when they hit send, use PHP to connect to a database (assuming you have a web-host) and have it automatically enter the information there.
