This is the screen to use if you need to have more flexibility in the content. While the author recommends using it only with screens you have control over, I have also successfully included Survey Monkey in apps. Then I put the survey results on a website that I created. Make sure that if you include a URL to a website you don't control that you test thoroughly. Works great on iOS and Android.
The alternative to this screen is to use the Launch plugin. Use this instead of the Launch plugin if you can because it keeps the user within your app.
